The pen refill may not extend fully. Usually there is a little more tolerance with the rollerballs but not with this style of pen.

You might be able to get about another 3/8 in length. However, for every bit of extension you try, you will have that much less gripping area on the end of the transmission. The top end of the pen is what holds onto the transmission.

yes you can. The refill is enclosed in the twist mechanism and it operation is not affected by the length of the barrel. But engaging the transmission will be. Remove the barrel and slip it back on just enough to engage the transmission. I just tried on and had 3/8 ths of an inch gap. so, I could lengthen the barrel by that much and also add extra tubing. Other sierra type kits may have different measurements.

Can you add length to the sierra tube (cut your own) without it messing anything up in how the pen functions?
 
You can lengthen the Sierra's as much as you would like by simply creating a new activator tube in the barrel . After your blank is turned , turn a plug that fits tightly in the barrel . In this plug simply glue in an 8 mm tube in the plug and glue the plug in the barrel , make sure you leave enough room to press the finial/clip assembly in . With this method you can create as long a barrel as you would like .
 
As Butch states it is very possible, I added 1/2 an inch by cutting the Sierra tube in half and soldering a piece of 10mm tube(Cigar size) in between. You can just as easily glue one with epoxie or ca. An alternative method to Butch's method is to drill out the cap to accept an 8 mm tube and epoxie it in place, after you have verified its proper length. The red pen in my signature line is done that way.

Butch's idea is great, but I wouldn't bother with all that glue! The Sierra refill is very common. If you can find another stock, mass-produced click pen which uses it, take it apart and save the plastic bit under the button. Just shove this into the actuator. The spring will hold it all in place. That gives you about an extra 10-15mm.
Now you have to lengthen the tube. I would cut the blank to the required length and drill through to about 7mm (as per a slimline). Then drill in from each end NEARLY all the way) to 9.2mm (the Sierra tube dia). So, you have a about 10-15mm in the middle which is only 7mm dia. Next, cut the tube in hlaf and stick one half into the blank from each end.
We left the bit inthe middl eof the blank at 7mm as this is big enough for the refill to pass and strong enough to hold the pen together considering the brass tube doesn't go all the way to to bottom.
 
Great info!!! and the easy is to use the longer click pen tubes to get a bit of longer body.

I have a couple applications for a longer body that should work -- thanks for the approach.

This would work for the click Sierras but not the Twist Sierras . The twist mechanisms require an 8mm tube to activate the transmission . Also I have a problem with extra pieces just floating around inside my pens (like the damn little metal pieces that customers always seem to loose) . I haven't given any thought to the click versions but I'm sure they could be done too .
I like Mikes idea of just adding the 8mm tube directly to the finial/clip assembly but the activator tube will be unsupported and I'm not sure of the stresses it would be subjected to , I tend to over engineer things . Another way would be to drill the front short the amount of extra length you want and glue in the activator tube directly into the body of the pen then drill the end of the blank just long enough to glue in a short piece of tube for the finial/clip to press into .
