I tried to print a shipping label from within Paypal for a small flat rate box and noticed they do not offer a choice for the new "$4.95 small flat rate box".

Actually they don't have any of the new rates listed so you are out of luck printing any labels at all.

I sent an email asking when they would add the new rates to their site and got this reply, which looks like they either didn't understand the situation or weren't going to update their site thus making their label printing pretty much useless.

Hello George, we appreciate your interest in the Large, Medium and Small Flat Rate Shipping options that are available through the United States Postal Service (USPS). Currently, PayPal Shipping does not offer those functionalities. However, as our business relationship grows with USPS, we will eventually have a more expanded menu of services to offer our customers. There is no timeframe as to when this may occur.
I understand your frustration regarding this matter and regret any inconvenience it might have caused you.

You can print straight from the USPS web site and purchase postage that way. If you are wanting to pay with your paypal account, you can download the Paypal browser plugin that will generate, and auto complete the credit card information, a one time use mastercard number that will withdraw the funds from your account.

Sounds like either USPS (more than likely) or Paypal doesnt want to upgrade the "plug & play" service component and there is no plan in the future to do so. This is not suprising since USPS has a tendency to make changes without understanding the impact on the interface or service component of its clients.

Sooooo, as Ralph suggested above, you can grab and reuse a service component.

Hope this makes sense in English LOL
At least you didnt get a reply from somewhere in Pakistan although the response you did get was rather cryptic to say the least.
