Just out of curiosity if I have a kit with a ballpoint refill can I buy a rollerball refill to change it out to? Or are there differences in the transmission?

Rollerball refills will not work in ballpoint pens. rollerballs need a cap to keep the ink from drying out so there is no transmission. I think gel rollerball refills do not need a cap but I would expect size difference in the refills would be a problem.

If that is the case then what refills for ballpoint are the smoothest writing. I have a customer that likes a cigar I made but wants a rollerball.
 
Try a gel refill that is Parker compatible. My everyday pen is a cigar with a gel refill and it writes at least as smooth as a Hauser rollerball; but not a Schmidt.
 
You can get gel refills for parker style refills. Be warned, they are a bit more expensive and they go through ink a bit faster. You might want to buy a pack of 6 and add them to the pen when you deliver it.

If you can find one locally or are willing to order online , the smoothest ballpoint refill I have ever seen are the Fisher Space pen refiles . They use a special kind of ink and the refills are pressurized , they will even write upside down , and they almost write forever (I'm still using one I have had for almost 10 years) , and they have a 100 year shelf life .
