Pocket full of ink

I have an Americana style pen with a Schmidt 888 F refill. At the end of the day I had a large ink spot on my shirt. I kept the pen upright and there is no ink on or in the pen. No signs of the pen leaking. Any ideas how this happened? This is the first rollerball pen that I have used myself. Any thoughts?

That's a twist right? Maybe you had the twist the wrong way and "drew" on yourself all night.
 
Sometimes the "snap" of the cap on the pen can cause ink to come off the tip (one of the reasons you don't see that many snap-on compared to screw-on). Those ink droplets can transfer to other parts of the pen and spread even further when the pen is snapped open.

Perhaps you had an errant droplet or something like that.

That's the only thing I can think of.
 
Sudden pressure changes are bad. A snap cap (vs a screw on or mag cap) can result in a sudden, large, pressure increase when putting on. And a sudden, large, pressure decrease when taking off.

I have one of the xFountain Pens snaps on my desk at work. Splatters frequently inside the cap. A rollerball is going to react much the same.
 
Is it possible that sometime during the day you had another pen in that pocket and that pen was the one that leaked? The fact that you could find no sign of leakage or ink in or on the pen has me stumped.
