Plans for a Toothpick Holder

Does anyone have the plans for making the Toothpick Holders? I had made one a while back for my dad. I showed it at the "Show & Tell" we have at the monthly meeting and one of the guys asked me if I had the plans. I didn't measure or anything just trial and error but it turned out really nice. So if anyone has the plans to make one I would like to get a copy.

sure is nice to see someone else do it instead of trying to decipher the written directions.
