This is the closest I could find that is a click with a parker refill. The fact that the tip doesn't have the usual shoulder most slimlines I've seen is what makes me think it's this kit.
As a side note, that's an interesting blank. Very chaotic. Hopefully you can help this woman fix her daughter's pen so she has something more to remember her with.

If you don't find the exact kit, you might be able to franken kit something that could work? What is wrong with it currently.

My first thought was the PSI Slimline Pro as it is a clicker that uses 8mm tubes and has a Parker style refill. The face of the clip would tell the story, but I can't see it in your pictures. The PSI has a black face on the clip. As Gregory (GFHWoodWerks) said, perhaps you can franken it. The tubes on the PSI are both 2-1/32" inch long.

Glad you figured it out and hopefully you can fix it up for her. Great that you are giving her memories of her loved one back. Might i suggest getting her a new refill too.

Just had to get a click mechanism for this kit from Craft Supplies. They were very helpful.
