Steve: Check with Ed at Exotic blanks and McKienzie Pen works Both are members of the form as they both have inlays.

Also member ochoabrian makes customer inlay blanks.

Ken Nelsen at Kalenshaan Woods will make custom inlays-but you need to order a small quantity (10 ish)
Making one is expensive--time to design, etc.

For single pens, you are better advised to find a "label caster" and provide him-her with the clip art. Then have it clear cast to make your pen.
