How are blanks like this cast? I've been trying to find out but come up with nothing yet... Any ideas?

Glad you asked. I always wondered myself.

I wonder if they make it like they do hard candies. They start by making a design in a huge log, and then keep pulling it until it is the right diameter. The design keeps getting smaller but doesn't change. Maybe they can do the same with plastic and then bake it hard when it is the right size? I assume some smart person out there knows how it's done.

You can do that with polymer clay, look up "millifiori". That's what Tori does to make her fantastic pen blanks.
